Health and Housing Navigation Services

At The Humanity Project, we believe that everyone deserves the dignity of stable housing and access to quality health care. That’s why we offer no-cost Enhanced Care Management (ECM) and Community Supports (CS) to eligible Medi-Cal members through our contract with Partnership HealthPlan of California. Our team of experienced health and housing navigators work one-on-one with individuals and families navigating some of life’s toughest challenges—being unsheltered, managing complex medical needs, addressing behavioral health conditions, or navigating reentry as justice-involved individuals. If you're feeling overwhelmed by your health care needs or housing situation, we’re here to walk alongside you. Together, we’ll create a personalized plan to help you access care, secure housing, and stay supported every step of the way.

About Us
The Humanity Project is a mission-driven case management and housing navigation organization based in Northern California. We specialize in serving individuals and families who are unsheltered or experiencing housing instability and complex health or behavioral health challenges—particularly those living in rural, underserved, and Indigenous communities.
With decades of combined experience in social work, medical case management, and systems navigation, our team brings deep knowledge of the barriers that vulnerable populations face. We’re committed to walking alongside each client with dignity, compassion, and cultural humility—whether they’re exiting incarceration, seeking recovery, or navigating the health care system for the first time.
As a proud partner of Partnership HealthPlan of California, we deliver Enhanced Care Management (ECM) and Community Supports (CS) under the CalAIM initiative. Our services are designed to help Medi-Cal members access the care, housing, and support they need to stabilize and thrive. From coordinating hospital discharges to finding safe housing, securing vital documents, and supporting behavioral health wellness, we believe in meeting people where they are—without judgment, and with real solutions. At The Humanity Project, we believe housing is health care and we’re here to help you find both.
Enhanced Care Management (ECM) ​
ECM provides specialized support for high-risk Partnership/Medi-Cal members. This includes a dedicated health navigator to assist with coordinating care and essential social services.
Our services can include:

Finding doctors & scheduling appointments.

Help you understand your medications and how to use them safely.

Setting up transportation for medical visits.

Coordination with post-hospital care.

Connect you to local food programs.

Assistance connecting to community programs

Enhanced Care Management
At The Humanity Project (THP) we are dedicated to assisting unsheltered or at-risk individuals and families from becoming unsheltered. We help eligible Partnership members with their housing search and securing affordable housing.
